REGISTRATION GUIDELINES
Below is the list of required documents that will need to be turned in to register a student:

1. Certified birth certificate (long form)
2. SC Immunization record (long white form)
3. Parent/Guardian photo ID (with current residence listed)
​4. Two current DATED proofs of residence (One proof MUST be a either a home tax receipt or a rental agreement; Other proof of residence can be current utility bill, voter’s registration card, car tax receipt, or a current, dated State supported document clearly indicating the name and address)
5. Copy of student’s Medicaid card (required for 4 year old CERDEP only)​
